REQUEST FOR FILES
We are happy to provide files for insurance, job, or other medical-related reasons. Your records are maintained in strict confidence. We will only release these files to you or your designee. No information will be released without your signed consent. The actual medical records, including X-rays, are the property of the physician. If you wish to obtain copies of your files, you must visit our office in person and sign a release form. A nominal fee may apply. Your files will then be mailed to you or your designee.
